That does seem odd especially sense custom printing was not easily done back
then by the average person.

It is fairly normal. They are (were?) produced by cutting from a large sheet.
Of course, some people might cut down a large one ro make a smaller one, especially
if it has damaged edges or corners.

My boss has had this plain yellow (non pearl) Star Wars C-3PO minifigure for
something like 20 years. He says he was told it was rare soon after he first
got it but knows no more information.

He's a bit of a technophobe so asked me to see if I can find out about it
but i haven't found anything, so wondered if someone here might be able to
reveal some information about it and if it has any value

You've got it backward. You don't add a color so you can add it to an
inventory. You add it to an inventory so it appears in that color.

Also, if the minifig you are working on comes from a set with printed instructions,
you should be working with the PCC (seven digit codes) from the list of parts.

28621 is an alternate number for 3626, which certainly has been issued in Black
in the past.
