Not a sprue stricto sensu but always with two other wheels.

BrickLink's definition of Extra parts could probably use a little updating:

What exactly is considered an extra has been the subject of significant contention
over the years, likely because the guidelines weren't precise enough. I'll
leave your request open for the moment, but it won't be forgotten.

You are correct that this part is not necessary to build the train. However,
it was indeed regularly included in the set as one part of a multipack and the
fact that it is not used in the construction of the set does not mean that it
should be considered an extra part.

Updating the Help page to be more specific about the precise definition of Extra
parts is definitely on the list of things to do.

Actually, I'd like to see them all moved to Gear where they belong by BrickLink's
own part/gear definitions. Then the whole regular/extra discussion for them
would lose some intensity - they're an item of Gear (a human tool) which
is regularly included in 400+ sets.

Sure. I could easily live with them being in the Gear section. However, the part
counts for inventories do not line up with part counts from official set boxes
when they are included in either the Extras or Gear section. I would like
to see this policy change so that part totals on set boxes line up with the part
totals in the inventories. Just my opinion. (This opinion includes having minifig
parts and animal parts all separated by default in the inventories with added
"assemblies" towards the bottom that could be used for part-out purposes.)

Ah, fair enough. I agree that it would be nice to have BL part counts match
official part counts whenever possible, but I imagine BL is still some ways from
making that happen - it's crazy how much work there is still left to do on
so many things.
