I am wondering if anyone else may have come across this sort of potential defect
where the level of the stud top is recessed slightly. Or might this be a dreaded
fake brick? It looks very normal in shape, color and texture to me otherwise.

I've seen this exact trait in knockoffs and bootlegs. never seen it with
an official part. Might want a second opinion though i'm not a professional.

I very much doubt it's fake - the sprue mark being so small is a characteristic
of Lego

Yeah, it's genuine Lego, sure as I can be without seeing it in person. I
have a few parts from my original '70s and '80s sets that have similar
studs, and there are those cool old Samsonite parts from the '70s, often
1x6 and 1x8 bricks, that have mixed studs. Along with varied orientations and
fonts, they often have varied amounts of depression of the stud just like the
pictured parts. Most aren't as deep as the pictured ones, but I've seen
a few that are.

Looks like it may have been heated and pressed.

It is genuine LEGO with a production defect. The top of the stud with the plastic
injection point was not positioned correctly into the mold.

I have to agree with WoutR that the part is genuine but flawed in production.
Does anyone have other examples in different parts with this same type of flaw?
