| Is it possible to add Items to a paid Order without creating a new order? when
I create a cart for the Missing Pieces I need from the same seller, when I go
to pay via PayPal it's charging me another extra postage and 20%UK tax on
top.

No, that's not possible.
The seller could refund the payment, change the payment and order status to none
and pending.
Then you can add a second batch to the order.

No, that's not possible.
The seller could refund the payment, change the payment and order status to none
and pending.
Then you can add a second batch to the order.

Would it be possible to change the payment status to "Not Yet Paid", order status
to Pending, and put in the original payment as a credit, to avoid having to refund?

The idea of instant payment-orders is that no order additions can be made.

You can't change it to Not Yet Paid; I've tried for this exact scenario.
It requires a full refund (not a partial either) for you to be able to change
the order to Not Yet Paid. It's a good system to protect the buyer. BrickLink
really just needs a way to have sellers allow additions to paid orders (it's
already done on a competing site). Not as critical of a feature as some others
though of course.

Sadly, that's a feature that's missing on BrickLink The only way to
add to an existing paid order like this (especially when the store uses Instant
Checkout like the one you're talking about), is to contact the seller directly
and ask if you can add to it. They may agree and have a manual checkout option
for you to use, or they may simply refund the extra shipping you pay on the 2nd
order.

Not really. If you decide to unwind the existing order, and have them place a
new order, that will cost you payment service fees (PayPal or Stripe). Buyer's
have to make their entire selection, and be done with it, before hitting the
pay button. To be able to add to an existing order, and to adjust the payment
upwards, would require changes to the BL ToS and changes to the interface with
the payment services. That would give sellers a capability, and a responsibility,
that would concern some buyers.
