I don't want to get negative feedback for something that is not my fault.

Probably best just to write it off if it's not too much money lost. Inform
the buyer that although it was not your fault, you will refund. If it's an
expensive part then ask for them to return it, if not don't bother. Better
to lose a few beans than get an unwarranted Negative feedback.

Yes, I have and we all run this risk when we list damaged items. Buyers can check
out with Easy Buy and never be shown the Item Description. Or, they just don't
notice. It isn't your fault, but it is the fault of our system more or less.
It's better to accept that it is an understandable miscommunication, refund,
and keep this buyer happy.

In my experience, negatives are more often a reaction to a poor response then
to a mistake. I would express understanding that they didn't get what they
had hoped for and are obviously concerned. Point out the item description once
and if that doesn't generate an immediate mea culpa, then refund and move
on.
When I get an order with a part that I have described as less than ideal, I proactively
email them asking to confirm if they read the description.
I do the exact same thing. I've only attempted to sell a few items that
were in less than ideal condition but after IC I reached out to verify they read
the description - most hadn't but still bought it. If they don't reply,
I send a print out of order, highlight the item description, and send that in
the package.
