If the buyer has already paid for the order, you can send a message to the buyer
letting them know about the missing item from your inventory and give them options
to refund the missing part OR cancel the order.
Don't be surprised if the buyer ignores the cancel item request. This is
what usually happens for me. If they don't respond at all, I just send the
order with an included cash refund for the missing item.

I wouldn't include cash. Do the partial refund properly so there is a record
of it and tax is handled properly.

You can't if the order is already paid, unless you do a full refund and change
the payment status of the order (which is probably more trouble than it's
worth). In these cases it's best to contact the buyer and suggest either
a partial refund for the missing item plus shipping (in case the buyer is obliged
to buy the item elsewhere) or a cancellation of the entire order.
