I added handling fees on all of my shipping methods a few days ago. The goal
is to charge a $1.29 Handling Fee on all orders, and charge an additional $5
handling fee on orders under $10. I have however received several orders since
that were under $10, but only the $1.29 Handling Fee was applied. I checked my
settings and everything is correct. I am assuming that either the shipping system
only allows one handling fee even though you are allowed to add multiple, or
the system is not working as intended. Here is a screenshot of the settings:

In your screenshot there's some text that mentions if multiple conditions
are met (which they are in that case), that the lowest will apply. It won't
allow you to do multiple.

Try:
Apply $6.29 USD when item total price is less than 10.00 USD
Apply $1.29 USD when item total price is over 9.99 USD
