Is it known if some belong to a specific set/time period or are they just random
variations?

I doubt anyone knows for sure, just as there is still a lot to do sorting out
which early minifigures had both solid and blocked open studs.
Many are listed with either one or the other but may have been both so require
an alternative head added to list of parts.
How that ever gets resolved is beyond me, exact time lines are not easy to establish.
There is a note somewhere stating it is still to be determined.

I wonder if before the digital age each printer had to be manually set somehow.
That's a wide variation.
