afaik it doesn't matter what buyers want or check, delivering the package
is always the seller's responsibility by law, so you'll always have to
pay for lost packages. Insurance is for you, not for the buyer.

Voted No.
You can already set this up in the shipping section of your store terms. You
have option for "by request" or "by default" with price bands.

Now for my rant...
Insurance protects the seller, not the buyer.
If you are worried about lost packages (or the $ value), you can purchase insurance.

Maybe I'm lucky... I've shipped thousands of packages via USPS and the
number lost could be counted on two hands (with some fingers left over).

If you give a check box, buyers will opt not to include it. If you insist on
it, set up specific shipping methods over a certain threshold.

Step 2a: charge for insurance over that threshold

You can already set up your shipping options this way. I think you can even force
buyers to pay for insurance if you want, or you could just bake it into your
S&H fees.
