It's probably this part, since rim and wheel usually came together at this
point in time:

Are we sure? I'd have said there was a considerable difference between two
parts coming pre-assembled, and an entire model coming ready-built 
Do we know for a fact that the wheel+tyre came like that in other sets?

This set is from 1976 and I know the sets such as
came with
but later releases of
and all 12v train sets such as
came with trainrims separate from train wheels.
So we know that this set 396-1 came with
but it is possible that later releases came with
not pre-assembled

This set is from 1976 and I know the sets such as
came with
but later releases of

Correction:
The 12v train sets such as
came first trainrims assembled to train wheels.
The trainsrims were separate from middle wheels
since late 1980, but still assembled to all four wheels of
[s=bb0012va]
Later (1981? 1982?) all trainrims came loose in any 12v or 4.5v train.
